Some NDIS participants are older Australians who entered the scheme before the NDIS–aged care interface transition. Support coordinators with aged care experience understand how to navigate the intersection of NDIS and Home Care Packages.
They can help participants access the right mix of supports regardless of which system funds them, and advise on what changes when participants turn 65 or transition to aged care services.
What to look for
